    TERM AND CONDITIONS

    1. ACCOUNT DELETION

      You are allowing “My Sanctuary Wellness Center, Inc. (SkinStation)” to retain the following data prior to
      your request of account deletion:

      1. Customer ID (Automatically-generated ID from SkinStation’s Data System)
      2. Name(Your Name Provided to our Branch/Online Platforms)
      3. Email Address (Your Email Provided to our Branch/Online Platforms)
      4. Mobile Number (Your Mobile Number Provided to our Branch/Online Platforms)
      5. Address (Your Address Provided to our Branch/Online Platforms)
      6. Purchase Records (Your purchase history to our Branch/Online Platforms, if there are any)
      7. Appointment Records (Your appointment history to our Branch/Online Platforms, if there are any)


      Except on the following reason/s:

      1. You don’t have Purchase Records in SkinStation in the past 10 years
      2. You don’t have Appointment Records in SkinStation in the past 15 years.

      SkinStation Customer Service will be contacting you through email to address your request and will be
      performing the following:

      1. If you don’t have remaining sessions on your purchased promo packages and/or services, our
        Customer Service shall:

        1. Deactivate your access on our online platforms (Mobile App, Websites and etc.)
        2. Disable notifications for transactional and marketing messages to your account.
      2. If you still have remaining sessions on your purchased promo packages and/or services, our
        Customer Service shall:

        1. Provide the amount to be refunded based on SkinStation’s Refund Policy
          (https://skinstation.ph/refund-policy/)
        2. Once your refund has been settled and you still wish to continue your account deletion,
          our Customer Service will deactivate your access on our online platforms and disable
          notifications for transactional and marketing messages to your account.
      3. If you don’t have any sale transactions and completed appointments, our SkinStation Customer
        Service will delete your account and remove all collected data on our platforms
    2. DATA RETENTION

      SkinStation are strictly following these local regulations in Republic of the Philippines

      1. Health Privacy Code Specifying the Joint A.O. No. 2016-0002, “Privacy Guidelines for the
        Implementation of the Philippine Health Information Exchange.” The Health Privacy Code states
        “Subject to existing regulations, all medical records, whether in electronic and/or paper format,
        shall be stored for fifteen (15) years.”
      2. Under RR 17-2013 and RR 05-2014, states “all books, registers, records, vouchers and other
        supporting papers and documents prescribed by the BIR must be kept by a business for a period
        of ten (10) years.”

    You agree that all data collected is retained electronically in maximum of 15 years from the last day of your
    engagement with SkinStation if you are not exempted as stated on Section 1. Account Deletion of this notice
